Reproductive outcome of single dose of corifollitropin-alfa comparing with recombinant FSH in advanced age women: a retrospective cohort study

  • Objectives: to evaluate reproductive outcomes of advanced age women candidate for IVF/ICSI stimulated with a single course of corifollitropin-alfa (CFα).

    Methods: An observational retrospective study was carried out at IVF unit of Federico II University from December 2012 to December 2015 comparing reproductive outcomes of 50 advanced age women stimulated with a single course of CFα (Group A) vs 100 matched controls where stimulation was performed with recombinant FSH (Group B). The following inclusion criteria were adopt-ed: age ≥ 35 years; Body Mass Index > 19 Kg/m2 and < 30 Kg/m2; basal Follicle Stimulating Hormone (FSH) < 12IU/L; menstrual cycles ranging from 24–35 days (intraindividual variability 3 days); hysteroscopic evidence of a normal uterine cavity. Woman affected by polycystic ovarian syndrome, genetic, immunologic and endocrinological disorders have been excluded. CFα was ad-ministered as a single subcutaneous injection, during the early follicular phase of the menstrual cycle in control group, starting dose of r-FSH was established evaluating BMI, age and ovarian reserve in terms of antral follicle count and was subsequently adjusted in function of ovarian response during follicular monitoring.

    Results: Both groups showed similar anthropometric, hormonal parameters. Duration and diagnosis of infertility were also comparable among groups. No statistical differences were observed between groups in terms of BPR and CPR. A higher number of follicles >15 mm was detected in Group A compared with Group B.

    Conclusions: According to our preliminary findings, in a subgroup of advanced age women, stimulation with a single dose of CFα is sufficient to achieve similar reproductive results compared to recombinant FSH. Larger trials in future could clarify which kind of women may benefit from this feasible approach.
